The Challenge

Pathologists and clinicians evaluating donated kidneys for transplant needed to review massive high-resolution microscopy slides and extract standardized clinical metrics — a workflow that demanded both medical-grade precision and true multi-tenant SaaS security.

The Approach

We built a Remix frontend with an advanced OpenSeadragon/IIIF medical slide viewer handling high-resolution TIFF images up to 2GB with deep zoom. The FastAPI backend, deployed serverless on AWS Lambda, uses a multi-tenant architecture with PostgreSQL row-level security via Supabase for organizational data isolation. Case and slide management covers presigned S3 uploads, draft reviews, and structured finalization, with an algorithm-driven analysis pipeline extracting clinical metrics like glomerulosclerosis percentage and sclerosed/non-sclerosed glomeruli counts, plus Supabase Auth with magic-link and institutional SSO.
